Current State Summary

Face / Head

Stable: Multiple fractures from the 2022 crash have structurally healed, but I still experience intermittent facial nerve irritation and sensitivity around the right cheek and temple. Flare-ups arise with overstimulation or travel but are generally stable. Headaches on and off but mostly stable with help of meds and injections.

Next: Continue monitoring; no new intervention expected.

Ear / Nose / Throat

Stable: Sinuses, facial reconstruction. Plates and bones appear to be stable.

Next: Persistent crusting at perforated septum and discharge as of 8/25, starting nebulized antiobiotic/steroid compounded med for one month (11/25), explore perforation repair surgery

Eyes

Stable: Post–orbital repair remains solid. Right eyelid repairs remain stable, eye closes fully, and dryness continues to improve.

Next: Exploring solutions to glare sensitivity with Denver Dry Eye and Neuropathic Pain Specialist.

Jaw / Bite / Mouth

Stable: Facial fracture repairs remain stable, sunises clear, oral and maxillofacial surgeries are stable, orthodontia work complete, dental work stable and complete. Nerve irritation and sensitivity as well as persistent numbness in lips, gums, teeth. Flare-ups arise with overstimulation or travel but are generally stable.

Active: Dental check ups to monitor health and stability.

Right Arm & Hand

Stable: Sensation has largely returned and fine motor control is improved, though endurance is still limited with prolonged tasks.

Next: No further surgeries or therapies planned.

Hips & Pelvis

Active: Right hip labrum repair, femoroplasty, and PAO are healing well and appear to be stable. Right hip is full weight-bearing and nearly off the cane, with gait retraining and glute/core integration continuing. Strength is improving weekly, though endurance remains limited.

Next: Left hip PAO + arthroscopy reconstruction scheduled for December; currently in pre-op preparation and conditioning mode.

Next: Continue strengthening and balance retraining ahead of left hip surgery.

Legs / Knees

Stable: Bilateral femur fractures.

Active: Gait is improving but still asymmetric due to continued right hip surgery healing. Knee pain and complications continue.

Next: Continue strengthening and balance retraining ahead of left hip surgery.

Ankles / Feet

Stable: left ankle tibia pilon fracture, right ankle syndesmosis repair

Next: left ankle complications post hardware removal, ongoing strengthening and stability work, neuropathic pain management

Active Care & Therapies

  • Taos PT 2x/week focused on surgical rehab (and prehab)

  • Home movement practice 3-4x/week for ~.5 hour

  • 2x/day home scar treatments for new right hip scars

Face:

  • 10/16/22: facial laceration, laceration of oral cavity, LeFort fractures at level I/II/III, bilateral orbit fractures, mandible fracture, in the surgeons words there was a "separation of face from skull" and fractures were "in such comminuted pieces it was hard to tell what bone came from where"

  • 1/19/23: evidence of midface collapse, elongated middle facial third, concave midface deformity

  • 11/27/23: saddle nose deformity with depressed nasal dorsum, scarring of left ala with asymmetric alar base. broad scar on nasal dorsum.

  • 10/11/24: exposed plate in nose - “vertical buttress plate attaching at the maxillary left lateral incisor, which is slightly more angulated, now protrudes into the left nasal cavity” - removed 12/6/24

  • Right eye:

    • 10/16/22: scar tissue pulling down lower lid, causing eye not to close

    • 10/27/23 eyelid closes but there is severe ectropion eyelid

    • 4/5/24 eyelid closes but continued severe ectropion

    • ectropion improved post 4/5/24 surgery

  • Mouth:

    • 10/16/22: maxillary hypoplasia, apertognathia

    • 11/27/23 still has malocclusion in the form of a crossbite

  • Right leg:

    • 10/16/22: open FEMUR fracture, right ANKLE syndomosis repair with tightrope fixation, laceration of ANKLE

    • 11/7/22: Heterotopic ossification (HO) near femur fx site (bone forms abnormally in soft tissue)

    • 8/2/24: HIP: torn labrum, partial tear of gluteus Maximus

    • 8/2/24: KNEE: fibrosis of Hoffa’s fat

    • 4/25: HIP: displaysia, rotated femur

  • Left leg:

    • 10/16/22: open FEMUR fracture, ANKLE: open fracture of proximal end of L tibia (plafond fx/pilon fx)

    • 8/2/24: KNEE: fibrosis of Hoffa’s fat, early signs of arthritis

    • 4/35: HIP: torn labrum, displaysia, rotated femur

  • Arm/hand:

    • 10/16/22: open fracture of shaft of left ulna, laceration of hand, central slip of left ring finger, traumatic arthrotomy of proximal interphalangeal joint of the left ring finger, ulnar digital nerve laceration left long finger

    • 6/5/23: non-union ulnar fracture

  • 10/16/22: Acute respiratory failure following trauma and surgery, bilateral pulmonary contusion, postprocedural pneumothorax
